To: Parent/Guardian of Students with Life-Threatening Conditions

Your child has been identified as having a Life-Threatening Condition. As you prepare for next school year, please take a few minutes to review the enclosed life-threatening packet as there are steps you will need to take over the summer months to ensure your child is ready to begin on the first day of school. Washington State requires children with lifethreatening conditions to have a written medication or treatment order on file with the school prior to attending school. This law, Substitute House Bill 2834, took effect on June 13, 2002.
Under the law, “life-threatening condition” means a health condition that will put he child in danger of death during the school day if a medication or treatment order and a nursing plan are not in place. The law provides that a child may not attend school in the absence of a medication or treatment order if the child has a life-threatening condition that might require medical services to be provided at school.
These forms must be completed and you must meet with our school nurse before your child can attend school. Here is a checklist of what needs to be done:
1. Make an appointment with your doctor. 2. Take paperwork to your doctor. 3. As the doctor to sign the medication, treatment or diet orders. 4. Be sure you sign and date the forms also. 5. Fill any prescription at the pharmacy. (As the pharmacist to put he label on the medicine, not the outside of the box.) 6. Schedule appointment to meet with school nurse in August. Bring all paperwork, medicines and equipment with you for your appointment.

Si su hijo ha sido identificado con una condición de Vida en Peligro, esta carta es para usted. Como usted se esta preparando para el próximo ano escolar, por favor tome unos minutos para revisar el paquete que incluimos para las Condiciones de Vida en Peligro, ya que hay algunos pasos que usted debe seguir durante los meses del verano, asegurándose que su estudiante esta listo para empezar el primer día de clases. El Estado de Washington requiere que los estudiantes con condiciones de vida en peligro en tener una orden de tratamiento o orden de medicamento por escrito en los archivos escolares antes de asistir a clases.
Bajo la ley de “Condiciones de Vida en Peligro” quiere decir una condición de salud que pone al estudiante en riesgo do vida o muerte durante el día escolar si una orden de tratamiento o medicamento y si un plan de enfermera no está en lugar. La ley proviene que un niño(a) no pueda asistir a la escuela en la ausencia de el plan de medicamento o orden de tratamiento si el niño(a) tiene una condición de amenaza de vida cual pueda requiere servicios médicos provenidos en la escuela.
Estas formas deben estar completas y usted necesita unirse con la enfermera escolar antes que su hijo(a) pueda asistir a la escuela.
Aquí esta una lista de lo que usted necesita hacer:
1. Haga una cita con su doctor. 2. Lleve los papeles a su doctor. 3. Pídale al doctor que firme las ordenes de medicación, tratamiento o dieta. 4. Este seguro que usted firme las formas con la fecha. 5. Finalmente cualquier receta de la farmacia. (pídale al farmaceuta que le ponga las indicaciones de las medicinas en los frascos, no en las cajas.) 6. Haga una cita con la enfermera de la escuela en agosto. Traiga todos los documentos, medicinas y equipo con usted cuando venga a la cita.
