Parents & guardians, please return this page signed. __________________________________________________________________________________ ENGLISH Authorization for Use of Artwork (for parents) I hereby authorize the Museum of Photographic Arts (MOPA) complete usage rights of the images of the artworks that my son or daughter has submitted for MOPA’s Youth Exhibition. As the child's parent or guardian, I fully acknowledge the release of image rights to MOPA, for use in all publication and promotional purposes, including printed materials, printed advertisements, and online, as well as display in the Museum for the duration of the exhibition or any of the exhibition’s satellite locations. I further agree that my son or daughter will not receive compensation for the use of the image by the Museum. I understand in agreeing to the above that the Museum of Photographic Arts is making no guarantee that my son/daughter’s entry will be used for any of the above purposes and that there is no obligation on the Museum to do so. ESPAÑOL Autorización para Usar el Material Artístico (para los padres) Autorizo por este medio al Museo de Artes Fotográficas (MOPA) el derecho total de usar las imágenes del material artístico sometido por mi hijo / hija para la Exhibición Juvenil de MOPA. Como padre o guardián del menor, reconozco plenamente la liberación de derechos de imagen a MOPA para su uso en todos los fines de publicación y promoción, incluyendo materiales impresos, anuncios impresos y en línea, así como la exhibición en el Museo o cualquiera de las ubicaciones satélite de la exposición. Además estoy de acuerdo que mi hijo o hija no recibirá compensación por el uso de la imagen por parte del Museo.
