Send in your Home Care Benefit Fund enrollment forms by May 31 to secure your health coverage! You recently received a packet of information about the Open Enrollment period for health coverage through the Home Care Benefit Fund. This is an urgent reminder that you must fill out the forms that were included in the packet and return them to the Fund by May 31 for health benefits that begin on or after July 1. You should fill out these forms even if you have not worked the 100 qualifying hours for two months in a row, so that when you do become eligible, you will be able to access your benefits quickly. You can submit your forms to the Fund by: • Using the postage-paid return envelope you received with them; • Completing them online through “My Account” at www.1199SEIUBenefits.org; • Faxing them to (646) 473-8878; or • Visiting us at a Fund satellite office (you can find a list of locations by visiting www.1199SEIUBenefits.org and clicking on “Contact Us”) or our main office at 330 West 42nd Street.

If you do not return these forms, you will have to wait until the next enrollment period, from November 1 to November 30. So don’t delay!
