FINAL FORMS ATHLETIC CLEARANCES Dear Parents and Student Athletes, We are excited to announce that the Lawrence Public Schools Athletic Department has partnered with FinalForms, an online forms system that will help us boost compliance and safety and reduce repetitive paperwork for families. FinalForms allows you to complete and sign required athletic participation forms from your computer, tablet or mobile device. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, saving you time if you have multiple children. Starting with the Fall 2019 season, you must register for a sport using Final Forms. The only form(s) that are required to be printed and handed in to the Nurse’s Office are the NYS Physical Form and mandatory/ medical forms completed by your Provider as needed. **Please note, if you currently have a valid physical on file in the Nurse’s Office, you do not need to hand it in again or get a new Physical until it expires (valid for 1 Calendar Year). However, Sports Registration must be completed with FinalForms online in order to be cleared to play a sport this Fall. Final Forms will send you automatic e-mail notifications once information is entered into the system. It will inform of when a physical is close to expiring and you will be able to see if your child has been cleared to play. We are asking that ALL parents of athletes use FinalForms. Please register at – https://Lawrence-ny.finalforms.com – and follow the prompt to create your account, create your son or daughter’s account and sign all the forms! Click on the Parent Playbook (attached) for additional guidance. If you require any support during the process, scroll to the bottom of the page and click “Use Support”. FINAL FORMS, pagina de web, PARA REGISTRARSE Y CUMPLIR CON NUESTRAS REGULACIONES Estimados padres y estudiantes deportistas, Nos complace anunciar que el Departamento de Deportes de las escuelas públicas de Lawrence se ha asociado con la pagina de web FinalForms, un sistema de formularios en el internet que nos ayudará a mejorar el cumplimiento y la seguridad y a reducir el papeleo para las familias. FinalForms le permite llenar y firmar los formularios de participación para deportes requeridos desde su computadora, tableta o telefono móvil.
