Oak Street Health is a rapidly growing company of primary care centers for adults on Medicare in medically underserved communities where there is little to no quality healthcare. Oak Street’s care is based on an entirely new model that is based on value for its patients, not on volume of services. The company is accountable for its patients’ health, spending more than twice as long with its patients and taking on the risks and costs of their care. Oak Street Health has been investing in communities since 2013 – providing much-needed primary care for tens of thousands of people.

Our Patients • Patients are Medicare eligible, many of who are also covered by Medicaid • Currently care for more than 50,000 patients

Our Centers • Each center sees between 2,000-4,000 patients • Centers include patient treatment rooms, an engaging community room with social activities, behavioral health services, social services, podiatry (some centers) and pharmacy (some centers)

Our Impact • Oak Street Health has reduced hospital admissions by more than 40%1 • Oak Street Health is a five-star quality healthcare practice according to national standards (top 10 percentile) • Nine out of 10 patients recommend Oak Street Health to family and friends
