DUMAS INDEPENDENT SCHOOL DISTRICT 2018-2019 Pre-Kindergarten Pre-Enrollment April 16-20th, 2018 8:00AM - 4:00PM Dumas /SD Enrollment Center 411 Miller (West side of Administration building)

The Dumas ISD Pre-Kindergarten pre-enrollment for the school year 2018-2019, for children who will be 4 years old on or before September 1, 2018 and who live in the Dumas ISD attendance area. Eligible students attend Pre-Kindergarten for½ day (either morning or afternoon). Eligibility

The child/family must meet one of the following eligibility criteria: • The child is an English Language Learner (ELL). • The family meets the free/reduced lunch income eligibility guidelines (verification of income -current Income Tax return, 1 years check stubs, food stamp letter, notarized letter from employer, child support letter, disability letter (if applicable) etc. • The family meets the criteria of "homeless". • The child is an active duty military dependent. • The child is or has been in foster care.

Required to Pre-Register Parents must bring the following to the enrollment center, when requesting pre-enrollment: 1. Original birth certificate or passport, residen_cy card, 194 Refugee papers, etc. 2. Child's Social Security card 3. Child's current immunization record 4. Photo ID of parent or guardian 5. Proof of income (current Income Tax return, 1 years check stubs, food stamp letter, notarized letter from employer, child support letter, disability letter (if applicable) etc. 6. Proof of residency (current electricity, water/gas bill or current/valid apartment lease with the parent's name) 7. Guardianship notarized affidavit from DISD (if applicable) 8. Military documents (if applicable) 9. Foster care documents (if applicable)

Las pre-inscripciones de Pre-Kinder de Dumas ISD para el ano escolar 2018-2019, para niiios que tendran 4 anos para 6 antes del lro de Septiembre, 2018 y que vivan en el area de asistencia de Dumas ISD. Los estudiantes elegibles asistiran a Pre-Kinder par½ dia (Ya sea manana 6 tarde).

Eligibilidad

El nino(a)/familia debe de cumplir una de las siguientes criterias de elegibilidad: • El nino/a esta aprendiendo Ingles coma Segundo ldioma(ELL). • La familia cumple con los lineamientos de elegibilidad de ingresos (verificaci6n de ingresos­ mas reciente Reporte de lmpuestos, talones de cheques de un ano, carta de estampas de alimentos, carta notarizada de empleador, carta de manutenci6n de las hijos y/6 carta de deshabilidad (si es applicable) etc. • La familia cumpie con la criteria de "Desamparado". • El nino/a es un dependiente de un miembro militar activo. • El nino/a esta 6 ha estado en un hogar substituto temporal. �

Requirimientos para Pre-lnscripci6n

Los Padres deberan taer lo siguiente al centro de registro, cuando so/iciten la pre-inscripci6n: 1. Acta de nacimiento original 6 psasporte, tarjeta de residencia, papeleo de refugiados 194, etc. 2. Tarjeta de Seguro Social del nino/a. 3. Tarjeta de Vacunaci6n al corriente del nifio/a. 4. ldentificacion con fotograffa del padre 6 tutor. 5. Comprobante de lngresos (Reporte de impuestos mas reciente, talones de cheques de un ano, carta de estampas de alimentos, carta notarizada del empleador, carta de manutenci6n de los hijos y/6 carta de deshabilidad (si es applicable} etc. 6. Comprobante de domicilio (recibo de electricidad, gas, agua reciente 6 contrato de renta con el nombre de los padres reciente. 7. Afidavit notarizada de custodia de DISD (si fuera applicable} 8. Documentos militares (si fuera applicable} 9. Documentos de hogar substitute temporal (si fuera applicable).
