CHILD DOES NOT NEED TO BE POTTY TRAINED! ONLY PARENT registering MUST attend You CANNOT register without ALL of the following required documents. o

Child’s Original Birth Certificate (must be 3 or 4 years old by October 1st 2019).

If you are the foster parent please bring the ID letter from DCP&P. If you are a legal guardian, you must provide legal documentation from a family court. ONLY parents and/or legal guardians can register their children.

o You must live in the city of Plainfield! Proof of Residence: Current Utility Bill (Electric, Water, Cable or Home Telephone) under either one of the parents’ names. o

If the utility bills are included in your rent and/or they are not in either of the parents’ name, bring an Affidavit of Resident form notarized and signed by the person that the utility bills come to or to whom you rent from and one of their CURRENT utility bills and any CURRENT official mail in either of the parents’ names (For Example: Pay stub, Bank statement, Credit card, Medical or Cell phone bill).

o Immunization Records- Immunization Record MUST have ALL REQUIRED SHOTS as per the state of NJ! Shots from another country must be translated by a doctor’s office before registration. o Most Recent PHYSICAL Exam – Physical exam results CANNOT be OVER a year old. The physical exam results must be documented on the Universal Child Health Record Form. Private childcare centers throughout Plainfield will provide (6 hours) FREE preschool services for the Pre-school Program during the 2019-2020 school year. Child Find: If you are concerned, your preschool child is developing or learning differently, you can call your district to request an evaluation for preschool special education and related services. ¡NINOS NO TIENEN QUE ESTAR ENTRANADOS A USAR EL BAÑO! SOLO UNO de los padres debe asistir a la matricula NO se puede matricular SIN LOS SIGUIENTES documentos requeridos

o Certificado de Nacimiento Original del niño (deben tener 3 años antes/para el 1ro de octubre del 2019). Si usted es el guardián con la agencia DCP&P tiene que traer la carta de identificación. Si es el guardián legal deberá presentar verificación de la corte. SOLAMENTE padres o guardianes legales pueden matricular a los niños.

o Usted tiene que vivir en Plainfield! Prueba de dirección: Recibos recientes de utilidad (Electricidad, Agua, Cable ó Teléfono de casa) en el nombre de madre/padre del niño. o Si estos recibos están incluidos con su renta y no están en el nombre de madre o padre, traiga un formulario de Declaración Jurada del Residente notariada (escrita en inglés) firmada por la persona a quien le llega los recibos y que haga constar que usted vive allí; además un recibo reciente de utilidad a nombre de esa persona. También traiga cualquier correo oficial que recibe en esa dirección reciente (por ejemplo; Colilla de pago, Estado de cuentas de banco, Recibos de; Médico, Tarjeta de crédito o Celular). o Registro de vacunas- ¡El Registro de vacunas DEBE tener todas las vacunas requeridas por el estado de NJ! Registro de Vacunas de otro país necesitan ser traducidas por una oficina de doctor ANTES de la matrícula. o Examen Fisico mas reciente – La fecha en el examen fisico NO PUEDE ser MAS de un AÑO atrás. Los resultados del examen tienen que ser documentados en el formulario siguiente; Universal Child Health Record Form. Centros de cuidado privados alrededor de Plainfield proveerán servicios (6 horas) GRATIS para preescolares en el programa durante el año escolar 2019-2020. Child Find: Si le preocupa que su hijo de edad preescolar se esté desarrollando o aprendiendo en forma diferente, puede llamar el distrito escolar para solicitar una evaluación para educación especial preescolar y servicios relacionados.
