PARENT INSTRUCTIONS TO REGISTER ATHLETE USING 8TO18 DASHBOARD SYSTEM Dear Parents, In order for your child to tryout or practice for a sports team you must first register them online to our Athletics Department. You will use an email and a password. It is important to remember your log in as you will use this to register for all sports. Go to https://swmavs.8to18.com/ •

Click on “Create an Account” and enter your own email and create your own password. (Please be sure to remember this password as you will use this for future registrations and will be used for each student.) You will use same account information each time you register. Click on “Begin Registration” “Select Activity” o Choose the sport your child will be trying out for/participating in. “Select Participant” o Add a New Participant (or choose your child once created) o All information on this page is for your student, i.e. cell phone, email. It is important to provide their information since AD and coaches will use this to communicate to them. o If you don’t have medical insurance type “None” o If you don’t have the account number type “None” “Roster Details” o You may be asked for t –shirt or short size o Height and Weight will be used for rosters “Primary Parent/Guardian Information” o Fill out the Parent information on the next page. “Physical Form” o If you have not completed a physical yet and need a form to take to the doctor you may download and print. o A doctor is coming to YES Prep Southwest on August 25th after school for a physical day for all athletes. Cost of physical is $20 cash only. o Remember your athlete must have a current physical to begin by 8/28. You will receive notifications 60/30/10 days prior to your athlete’s physical expiring and day of. o

“Legal Form” o At this time by checking the boxes, you are agreeing and consenting to all information provided. o You must click on the form to read. Please note that when there is a parent/guardian and student check box- they must both be checked to move forward. o Please DO NOT turn in any forms filled out.

Fee Payment – You have the option to pay the student athletic fee online via debt or credit card. If you wish to still send the payment (Cash or Check) by your athlete like in the past you can as well. •

“Summary” o At this time you can see what you have registered for. o Click on “Finish” to complete your registration o You will receive an email confirmation upon registration submission
