PLEASE BRING YOUR CHILD TO REGISTRATION Speech, Language and Hearing Screenings will be conducted at the time of registration. Please bring an original state certified birth certificate with the state seal, as per requirement of WV State Law. Information for acquiring West Virginia documentation can be obtained at your county court house. Also, please bring proof of residency and a social security card. 1 of 2
EVERY CHILD ENROLLING MUST HAVE A COMPREHENSIVE HEALTH SCREENING as well as proof of a DENTAL EXAMINATION COMPLETED Health Check Forms are available at local physicians’ offices, the Jefferson County Board of Education office and the Pupil Services webpage, under Medical Forms. There is no specific form for the dental exam – at a minimum, proof of exam should show the date of the exam (must be within the previous 12 months) and have the dentist’s signature.
REQUIRED IMMUNIZATIONS Please be prepared to show proper documentation. -DPT – Four doses required. One dose must be after the 4th birthday. -Polio – Three doses required. One dose must be after the 4th birthday. -Measles, Mumps & Rubella (MMR) – Two doses required. First dose must be after the 1st birthday. -Varicella (Chickenpox) – Two doses required. First dose must be after the 1st birthday. -Hepatitis B – Three doses required. Last dose must be after the age of 6 months. -Tuberculin Test - Only required for students in high risk categories. See Jefferson County Health Department for screening information. *
To find out how to get immunization records, contact your family physician or contact the Jefferson County Health Department at 304-728-8416. The Health Department holds monthly immunization clinics.
A child must be 5 years old before September 1st to enter Kindergarten School policy allows admission of children born between September 1st and September 15th inclusive only if they meet eligibility requirements. To be eligible for early Kindergarten entrance, the child must score at or above the ninety fifth percentile on a standardized comprehensive measure of developmental skills when compared to age norms appropriate to a child who would be 5 years old prior to September 1. This assessment will be administered by qualified Jefferson County personnel. To enroll a child born between those dates, please contact the Pupil Services Department at 304-728-9236. Por favor traiga un certificado de nacimiento estatal original con el sello del estado, ya que es requisito por Ley Estatal de WV. En la corte judicial puede obtener información de cómo adquirir documentos de West Virginia. También, por favor traiga prueba de donde reside y tarjeta de seguro social.
TODO NIÑO INSCRIBIÉNDOSE DEBE TENER UNA EVALUACIÓN COMPRENSIVA DE SALUD y también evidencia de un EXAMEN DENTAL COMPLETADO Los Formularios de Chequeos de Salud están disponibles en las oficinas de médicos locales, la oficina de la Junta de Educación del Condado de Jefferson y el sitio web de Servicios Estudiantiles (Pupil Services), bajo Formularios Médicos (Medical Forms). La política escolar permite la admisión de niños nacidos entre el 1ro de septiembre y el 15 de septiembre solo si ellos cumplen con los requisitos de elegibilidad. Para ser elegible de entrar temprano a Kindergarten el niño debe obtener un porcentaje de noventa y cinco o más en una medida integral de destrezas de desarrollo cuando es comparado con normas apropiadas a la edad, en un niño que haya cumplido 5 años antes del 1 de septiembre. Esta evaluación será administrada por el personal cualificado del Condado de Jefferson.
