ETHS ATHLETIC REGISTRATION STEPS for completing the Athletic Registration process at ETHS: 1. Parent/Guardians or Students login to myETHS.com account using ETHS HAC credentials. If you don’t know your HAC credentials, call 847-424-7121. 2. Under “Records” select “Athletic Registration” in order to verify the following: a. Parent/Guardian Consent – consent must be given in order to complete registration. Consent can only be given through the Parent/Guardian account. b. Media Consent – without consent, student-athletes cannot be included in team pictures. Consent can only be given through the Parent/Guardian account. This is not required to register for sports. c. Doctor’s Physical Date – a doctor's physical is only valid for 395 days. The student’s physical should be submitted to the Nurses' Office right after the exam so the student-athlete remains eligible for participation in sports. It is most helpful to bring the State of Illinois Child Health Certificate to the annual physical. The Nurses' Office (N121) is open between 7:30am-4:00pm during the school year. Summer hours are June 6 – July 19, from 7:45am-1:35pm (closed Fridays and from July 23-29, 2018). The Nurses' Office is open the week of July 30 from 8:00am-4:00pm, but remains closed on Fridays through Aug. 10. Physicals may be faxed to 847-424-7254 or scanned and emailed as an attachment to [email protected] (camera photographs are not valid). NOTE: it takes up to two school days to process physicals so please plan accordingly. d. ImPACT Baseline Test – every freshman, junior or new student-athlete must have a valid test score. Register at www.eths.k12.il.us and click “Athletics” then “ImPACT Test Registration”.

3. Select the Season and then the Sport. Then click the green box “Register for Selected Sport” (if the box is red, the Sports Physical is expired or Parental Consent has not been given). Please register for all next year’s sports by selecting each season/sport one at a time.

If the registration process is not completed before the start date listed below for the sport in which you wish to participate, the student-athlete must visit the Athletic Office* to obtain a PINK SLIP to give to their coach. INSCRIPCIÓN A DEPORTES EN ETHS PASOS para completar el proceso de Inscripción a Deportes en ETHS 1. 2.

Los Padres/Tutores o Estudiantes ingresan a la cuenta de myETHS.com usando las credenciales de ETHS HAC. Si usted no sabe sus credenciales de HAC, llame al 847-424-7121. Bajo “Registros” (“Records”) seleccione “Inscripción a Deportes” (“Athletic Registration”) con el fin de verificar lo siguiente: a. Consentimiento del Padre/Tutor – El consentimiento debe ser otorgado con el fin de completar la inscripción. El consentimiento solo puede ser otorgado mediante la cuenta del Padre/Tutor. b. Consentimiento para los Medios – Sin este consentimiento, los estudiantes-deportistas no pueden ser incluido en las fotos del equipo. El consentimiento solo puede ser otorgado mediante la cuenta del Padre/Tutor. Esto no es requerido para inscribirse en deportes. c. Fecha de Examen Médico del Doctor – Un examen médico del doctor es válido solo por 395 días. El examen médico del estudiante deberá ser presentado a la Enfermería inmediatamente después del examen para que el estudiante-deportista permanezca elegible para participar en deportes. Es más útil traer el Certificado de Salud del Menor del Estado de Illinois State of Illinois Child Health Certificate al examen médico anual. La Enfermería (N121) está abierta entre las 7:30am-4:00pm durante el año escolar. Luego presione la caja verde “Inscríbase al Deporte Seleccionado” “Register for Selected Sport” (Si la caja es roja, el examen médico está expirado o el Consentimiento del Padre no ha sido entregado).
