Free Soccer Clinic The Dalton State RoadRunners, FSG Bank, Dalton Housing Auth & The Boys and Girls Club of Dalton have teamed up to provide a FREE soccer clinic for interested young people in our area. There will be free T‐shirts & soccer balls while supplies last. Hot Chocolate and water will be available. Who: Any child between the age of 8 and 18 What: Dalton State Roadrunners coach and team members would like to teach some basic skills to interested young people When: Tuesday, December 13,2016 from 4 to 5:30pm Where: Dalton Boys & Girls Club field behind the club located at 1013 Underwood Street

Requirements: Everyone par cipa ng must provide a waiver form signed by your parents or guardians!!!!  INJURY RELEASE/WAIVER - Soccer Clinic I, ______________________________the above named participant, have voluntarily applied to participate in the above activity. I acknowledge that the nature of the activity may expose me to hazards or risks that may result in personal injury and I understand and appreciate the nature of such hazards and risks. In consideration of my participation in the above activity, I hereby accept all risk to my health and of my injury that may result from such participation and I hereby release the hosting field, employees and representatives from any and all liability to me, my personal representatives, estate, heirs, next of kin, and assigns for any and all claims and causes of action for loss of or damage to my property and for any and all illness or injury to my person, that may result from or occur during my participation in the activity, whether caused by negligence the hosting field, employees, or representatives, or otherwise. I further agree to indemnify and hold harmless the hosting field, employees, and representatives from liability for the injury of any person(s) and damage to property that may result from my negligent or intentional act or omission while participating in the described activity. I HAVE CAREFULLY READ THIS AGREEMENT AND UNDERSTAND IT TO BE A RELEASE OF ALL CLAIMS AND CAUSES OF ACTION FOR MY INJURY OR DAMAGE TO MY PROPERTY THAT OCCURS WHILE PARTICIPATING IN THE DESCRIBED ACTIVITY AND IT OBLIGATES ME TO INDEMNIFY THE PARTIES NAMED FOR ANY LIABILITY FOR INJURY OF ANY PERSON AND DAMAGE TO PROPERTY CAUSED BY MY NEGLIGENT OR INTENTIONAL ACT OR OMISSION.

Clases Gratuitas de Futbol Los Correcaminos de Dalton State, El Banco FSG, La Autoridad de Vivienda de Dalton, y el Club de Niños y Niñas de Dalton Se han reunido para proveer una clase GRATIS de futbol soccer para los jóvenes in‐ teresados en nuestra área. Habrá camisetas y pelotas de futbol gra s hasta agotar. Quien: Cualquier niño entre la edad de 8 y 18 años Que: Los Correcaminos de Dalton State y miembros del equipo quieren enseñarles habilidades básicas a jóvenes interesados. Cuando: Martes 13 de Diciembre 2016 de 4 a 5:30pm Donde: En la cancha de Dalton Boys & Girls Club detrás del club localizado en el 1013 Underwood Street  AUTORIZACION DE LESION/EXTENCION‐Clase de Futbol Yo,_______________________el par cipante, he aplicado voluntariamente para par cipar en la ac vidad anterior. Reconozco que por naturaleza de la ac vidad puedo exponerme a peligros o riesgos que pueden resultar en daños personales y en endo los peligros y riesgos. Teniendo en cuenta que por la par cipación en dicha ac vidad, con la presente acepto todos los daños a mi salud y lesiones que puedan resultar de tal par cipación, y por la presente libero a los organizadores, empleados, y repre‐ sentantes de cualquier y toda responsabilidad mía, y de mis representantes personales, propiedad, herederos, familiares, y asignar cualquier y todos los reclamos y causas de la acción por la perdida de o daños a mi propiedad y por cualquier enferme‐ dad o daño a mi persona, que pueda resultar de o ocurrir durante mi par cipación en la ac vidad, aunque sea causada por negligencia de los organizadores, empleados, o representantes, u otra cosa.
