DUMAS INDEPENDENT SCHOOL DISTRICT 2019-2020 Pre-Kindergarten Pre-Enrollment April 15-18, 2019

8:00AM – 4:00PM Dumas ISD Enrollment Center 411 Miller (West side of Administration building)

The Dumas ISD Pre-Kindergarten pre-enrollment for the school year 2019-2020, for children who will be 4 years old on or before September 1, 2019 and who live in the Dumas ISD attendance area. Eligible students attend Pre-Kindergarten for ½ day (either morning or afternoon). Eligibility

The child/family must meet one of the following eligibility criteria: • Unable to speak or comprehend the English Language • Is eligible to participate in the national free or reduced-price lunch program (verification of income –current Income Tax return, 3 check stubs, food stamp letter, notarized letter from employer, etc.) • Homeless • The child is an active duty member of the armed forces of the United States, including the state military forces or the reserves • The child of a member of the armed forces of the United States, including the state military forces or the reserves, who was injured or killed while serving on active duty • is or has been in foster care • The child of a police officer, firefighter, or emergency medical responder seriously injured or killed in the line of duty and received the Star of Texas Award

Required to Pre-Register

Parents must bring the following to the enrollment center, when requesting pre-enrollment: 1. Original birth certificate or passport, residency card, I94 Refugee papers, etc. 2. Child’s Social Security card 3. Child’s current immunization record 4. Photo ID of parent or guardian 5. Proof of income (current Income Tax return, 3 check stubs, food stamp letter, notarized letter from employer, etc.) 6. Proof of residency (current electricity, water/gas bill or current/valid apartment lease with the parent’s name) 7. Guardianship notarized affidavit from DISD (if applicable) 8. Military documents (if applicable) 9. Foster care documents (if applicable)

Las pre-inscripciones de Pre-Kinder de Dumas ISD para el año escolar 2019-2020, para niños que tendrán 4 años para ó antes del 1ro de Septiembre, 2019 y que vivan en el area de asistencia de Dumas ISD. Los estudiantes elegibles asistirán a Pre-Kinder por ½ dia (Ya sea mañana ó tarde).

Eligibilidad

El niño(a)/familia debe de cumplir una de las siguientes criterias de elegibilidad: • El niño/a esta aprendiendo Inglés como Segundo Idioma(ELL). • La familia cumple con los lineamientos de elegibilidad de ingresos (verificación de ingresosmás reciente Reporte de Impuestos, 3 talones de cheques, carta de estampas de alimentos, carta notarizada de empleador, etc.) • La familia cumple con la criteria de “Desamparado”. • El niño/a es un dependiente de un miembro militar activo. • El niño/a esta ó ha estado en un hogar substituto temporal.

Requirimientos para Pre-Inscripción

Los Padres deberán taer lo siguiente al centro de registro , cuando soliciten la pre-inscripción: 1. Acta de nacimiento original ó psasporte, tarjeta de residencia, papeleo de refugiados 194, etc. 2. Tarjeta de Seguro Social del niño/a. 3. Tarjeta de Vacunación al corriente del niño/a. 4. Identificación con fotografía del padre ó tutor. 5. Comprobante de Ingresos (Reporte de impuestos más reciente, 3 talones de cheques, carta de estampas de alimentos, carta notarizada del empleador, etc) 6. Comprobante de domicilio (recibo de electricidad, gas, agua reciente ó contrato de renta con el nombre de los padres reciente. 7. Afidávit notarizada de custodia de DISD (si fuera applicable) 8. Documentos militares (si fuera applicable) 9. Documentos de hogar substituto temporal (si fuera applicable).
