Dear Parents, We would like to inform you that for School Year 2019-2020, only special diets that are ordered for diagnoses or conditions that are considered disabilities will be accepted. This must be noted on the special diet prescription form. It is the responsibility of the parent to ensure that the medical statement provides an explanation for the student's disability and how it restricts the student's diet. On average, special diets take two to three weeks to be processed and completed. The dietitians may need to contact you or the physician to clarify any questions related to the diet order. It is possible that it may take longer to process the diet based on severity and/or whether or not the special diet prescription form is filled out in its entirety. During the time the diet is being processed, your child will be receiving regular cafeteria meals. If you prefer that your child not receive the cafeteria meals, please make arrangements to provide meals for your child. Once a special diet form is submitted, only a physician may alter or cancel the diet. The cafeteria manager will advise you once the menu is ready. Esto debe ser anotado en la forma de dieta especial. Sera la responsabilidad del padre asegurar que una explicación de la discapacidad del estudiante y como restringe la dieta sea documentado en la declaración medica . El promedio para procesar y completar el menú de las dietas especiales es entre dos y tres semanas. Puede ser que los nutriólogos hablen con usted ó el medico para clarificar alguna pregunta. Es posible que se tarde un poco mas el proceso dependiendo de la severidad ó si la forma de las dietas especiales no esta llena completamente. Durante el procedimiento, su hijo/hija va a recibir comida regular de la cafetería. Si prefiere que su hijo/hija no reciba comidas regulares de la cafetería, favor de hacer arreglos para proveerle comidas a su hijo/hija. Cuando usted entrega la forma de la dieta especial, solo un medico ó asistente de medico pueda cambiar or cancelar la dieta. La gerente de la cafetería estará en contacto con usted cuando el menú este listo.
