High Pines / Ponce-Davis Area Annexation Survey & Petition Form Dear Voter: The City of Coral Gables is sending this form to all registered voters in the High Pines / Ponce-Davis area in order to gauge community support for annexation of your area into the City. For more information regarding the cost of annexation to property owners, police and fire response, and other services the City will provide with annexation, please visit www.coralgables.com/annexation or contact the City at 305-460-5248. The general boundaries of the High Pines / Ponce-Davis area are: North – SW 72nd Street and San Ignacio Avenue East – SW 47th Avenue and Mindello Street South – SW 88th Street West – SW 57th Avenue. (See proposed annexation area map on next page) Please mark the appropriate box below as to whether you are in favor or not in favor of Coral Gables submitting an application to Miami-Dade County to annex your area into the City, sign the form and provide the other information requested. Then place the completed form in the enclosed self-addressed, stamped envelope, and mail back to us. Favorable responses will help us satisfy Miami-Dade County Code Provision 20-3(H), which requires that signed support petitions from at least 20% of the registered voters in this area be submitted with the City’s annexation application.
