Copy of Child’s Birth Certificate (must be 5 on or before Sept. 1, 2018) Copy of Child’s Social Security Card Immunization Certificate (Form 3231) Certificate of Vision, Hearing, Dental, and Nutrition Screening (Form 3300) Proof of Residency – 1 required from each type below: (Documents must show parent/guardian’s complete name and physical address.)

Type 1 : House Closing Papers, Deed, Mortgage Documents, Rental Agreement/Lease, most recent income tax return, current residential property tax statement or bill, current paycheck stub

~AND~ Type 2 : gas, electric, oil, water, cable or home phone bill (NOTE: Cell phone bills will not be accepted as valid proof of residence.)

A Third Party Affidavit may be signed on behalf of the child who wishes to enroll in a Tattnall County School. The Third Party Affidavit will be accepted along with the required documentation from Type 1 and Type 2 proving Tattnall County residency of the Third Party.

 NOTE: If possible, please bring child to registration. All documents must be original; however, registration will be expedited if applicant provides a copy of requested documentation listed above. Currently enrolled Pre-K students at CES, GES, RES, or TCHS are not required to register.

Tipo 1: documentos de cierre de contrato de la casa, escritura, documentos hipotecarios, contrato de alquiler/arrendamiento, declaración de impuestos más reciente, declaración o factura actual de impuestos de propiedad residencial, comprobante de cheque actual

~ Además necesita ~ Tipo 2: factura de gas, electricidad, petróleo, agua, cable o teléfono residencial (NOTA: las facturas de teléfonos celulares no serán aceptadas como prueba válida de residencia.)

La declaración jurada de un tercero puede ser firmada en nombre del niño que se desea inscribir en la escuela del Condado de Tattnall. Se aceptará una Declaración Jurada de Terceros junto con la documentación requerida de Tipo 1 y Tipo 2 que demuestre que la tercera persona es residente del Condado de Tattnall.

 NOTE: Por favor, si es posible, traiga al niño/a a la inscripción.

Todos los documentos deben ser los originales; sin embargo, la inscripción será más rápida si usted trae una copia de los documentos mencionado arriba. Los estudiantes que están registrados en Pre-Kínder en CES, RES, GES, o TCHS este año escolar, no requieren una nueva inscripción.
