California-Pacific Justice & Compassion Essential Ministry Team Block Grant Funding Request for Calendar Year 2013 Individual Ministry and/or Project Priorities: The Justice and Compassion Essential Ministry Team supports a wide variety of new and ongoing programs and projects that are connected to the people of the United Methodist Church. Please be aware that of the four areas of ministry identified by the Council of Bishops we will be focusing on engaging in ministries with the poor and combating the diseases of poverty. However, we are aware that all four areas of focus often overlap, so please identify how your project will help revitalize your congregation or develop leadership for the church. We also want to encourage and give preference to cooperative ministries, i.e., those involving several UM churches or Christian churches of several denominations. Please be aware that the block grants given in 2013 will be less than $3,000 per grant. Given the reduced size of the grants, grants will only be given to organizations with annual budgets less than $500,000 with priority given to smaller agencies.

Proposal Package – the following requirements are designed to help in reducing costs and staff/volunteer time in completing the application. Proposals that do not adhere to the following requirements may be rejected.  Submit one original and 3 complete copies of the proposal package to your District Office.  All proposal materials must be submitted together; if not, the proposal may not be funded.  Do not submit elaborately bound proposals; paper clips are adequate.  Minimum font size: 12 point, with minimum 1-inch margins. Most applications, including attachments, should not exceed 8 total pages.  Materials will not be returned. Please, no videos, newspaper articles, CD’s, DVD’s, or other information that is not requested. 1) Proposal Cover Sheet (Attached) The completed cover sheet must be signed by two of the following: the presiding officer of the board, the executive director of the organization, a primary volunteer, or the pastor. This is to indicate that both the board and the primary organizing staff person have approved this proposal AND that all requested documents have been included in the request. Applications submitted without two original signatures will not be considered. 2) Proposal (3 pages only) – please address each of the following using each question/request as a heading for each response.  

Please describe the specific project including its activities and mission/ purpose. How does your proposal engage the poor or combat the diseases of poverty?

   

What connection does the project have to a particular United Methodist Church of the California-Pacific Annual Conference? Does the project have community support beyond a particular UMC? Please describe that support. How are you working together with other churches, whether Methodist or of other denominations, to develop this ministry? What is the total budget for this project and what is the amount of the grant you are requesting? Specifically, for what will the requested funds be used? What are your major sources of funding? What adjustments to the project will you make if you receive only partial funding from this conference board or if you receive no funding from us whatsoever? If this is a new project, why do you think it is necessary? If it is an ongoing project, what success have you had in prior years in meeting your objectives? Please give the qualifications of the leadership, staff, and volunteers who will be responsible for the project. How will you measure the success of your project or what criteria will you use to evaluate it? Have you applied for and/or been awarded grants from any other UM source for 2013? Please identify the other source and the amount requested or awarded there.

3) Additional Materials  Organization’s balance sheet. (One page only, please.)  Total project or organization budget with revenues and expenses. (Again, one page.)  Current list of members of the organization’s board of directors or primary volunteers, with occupations or positions included. Collaborative Project Requests If the proposal is a collaborative effort, identify the other partners, their contributions to the project, and their qualifications to carry out these activities. Report of Grant Expenditures All grant recipients are required to submit an interim report by August 1, 2013, and a final report by February 1, 2014 to the JCEMT staff at the annual conference. Failure to submit a timely interim or final report may result in the denial of future grant requests. Deadlines This grant application must be postmarked no later than December 15, 2012, to your District Office. Late applications will not be considered. Unfortunately, more funds are requested than there is funding available, and there is no guarantee that any particular project will be funded.
